Welcome to the Northern Kentucky University Police Departments' on-line crimes report form. If this is an EMERGENCY or the crime you wish to report is in progress call the NKU Police Department at "911" or "572-7777". If you want to advise the Police Department of criminal activity in your dorm, building, or work area you have reached the right place. All you have to do is fill out the form below and it will be sent directly to the Police Department.

INSTRUCTIONS:

1) This form is only being used at this time for those individuals of the Northern Kentucky University Community that for whatever reason wish not to inform the Police Department in person.

2) Please use this form only to report crimes that occurred on property owned, rented, or leased by Northern Kentucky University.

3) Please complete as many lines of this report as you possibly can.

Describe what happened	Give a detailed description of what happened. The more information the better.
Description of Suspect	Please give a complete description of suspect. Include name, race, age, height, weight, work address, home address, and/or any other information you can.
Description of Vehicle	If a vehicle was used in the commision of this crime, please give a full description: license number, license state, vehicle make, model, type, color, or any other indetifying marks.
Investigation	If you wish to be contacted by the investigating officer, please fill out the e-mail address field. Contact will be made by sending you an e-mail message and setting up a time to be interviewed.
