DENTAL DEDUCTIONS
Premium Contributions: 2008 - Delta Dental
Dental coverage is provided and underwritten by Delta Dental Plan of Kentucky. Dental benefits begin on the first of the month following 30 days of employment. Rates are guaranteed through December 31, of the year. All eligible employees must complete an application with the provider to be covered. There are two different plans to choose from Preferred or Premier. The Preferred plan is a model that has in-network and out-of-network levels of benefits depending on whether the dental provider participates. The Premier is a traditional open network and provides one level of benefits regardless of participati

The University currently pays for single level coverage for the preferred plan.

Premium Contributions: 2008 - Humana We understand the need for you to evaluate costs when making a decision on your healthcare needs, Humana’s Plan Professor is a great tool to will help you evaluate the different plan options with a personalized cost analysis on items such as copays, coinsurance amounts, payroll costs, deductibles and many other important variables that are all part of your decision on healthcare. Remember, you can off set any out-of-pocket costs with your flexible spending account or health spending account through FEBCO – even more ‘user friendly’ in 2008 with the added debit card feature. Beginning in January 2008, all benefit deductions for bi-weekly employees will be taken on a 26 pay period basis. Non-active faculty and COBRA rates may be obtained by contacting Human Resources at 859.572.5200. Below please find the 2008 employee payroll contribution rates for the plan options available with Humana:
*There is an additional plan (Classic 250) available for employees residing outside of the tri-state area.
