TO ALL NEW FACULTY & STAFF EMPLOYEES
Welcome to NKU! In order to help you through all of the paperwork new employees must complete for tax purposes, benefit enrollment, federal regulations, etc., the Human Resources Department has compiled a packet of basic forms.
All new employees must come to the Human Resources Department no later than their second day of employment to complete the forms included in this packet. The completion and/or verification of these forms comply with federal and state laws. These forms also provide the information needed to place new employees on the payroll. Human Resources cannot process payroll documents until these forms are completed.
The forms contained in the New Hire Packet and their purpose are as follows:

New Employee Information - Please complete electronically, sign, print, and return to Human Resources. The form includes social security number, legal name, date of birth, citizenship, country of permanent residency. Complete this form so accurate reporting of withholdings can be made. Emergency contact information is also included on the Personal Data Sheet.
Federal and State Tax Forms, and Cincinnati tax, if applicable, also non-residency form. Complete these forms so the University can withhold the correct federal and state income tax from your pay.
Direct Deposit - All Northern Kentucky University employees are eligible to sign up for direct deposit. Direct Deposit Authorization forms are due in HR/Payroll 10 days prior to payday to become effective with the next payroll check.
Employment Eligibility Verification Form (I-9) - provides certification the employee is eligible to work in the United States. This form must be completed (by law) within three working days of the employee’s start date. Documents must be presented by the employee that establish both identity and employment eligibility. Examples of acceptable documents include, but are not limited to:
Driver’s license and social security card
Driver’s license and certified copy of birth certificate
United States passport
I.N.S. Documents reflecting alien registration

Office of Affirmative Action/Multicultural Affairs, Employees with Disabilities Status Report and Employee Veterans Status Report - Submission of this information is voluntary.
New employees must bring their Social Security card for review by HR so we can verify your correct name and social security number. This is necessary in order to accurately report your earning to Social Security.
New staff employees, eligible for benefits, will also be scheduled to attend New Employee Orientation. An overview of the total benefits package and specific plans will be discussed. The employee is given the opportunity to enroll or waive coverage.
An orientation for new faculty is held at the beginning of each academic year. At other times throughout the year, orientation is conducted by individual appointment.
