Appendix C

New Minor/Area of Concentration/Track,
Change/Deletion of a Program or Addition of Web-Based Program Option

When should I use Appendix C?

If proposing a new minor
If proposing a new Area of Concentration
If proposing a new Track
If adding a web-based program option to existing approved program

If proposing to delete a program (major, minor, area of concentration, or track)
If proposing to change a program (major, minor, area of concentration or track)

When should I not use Appendix C?

If proposing a new major or degree program - use Full Program Proposal
If proposing a new certificate program - use Certificate Program Proposal
If proposing a new web-base program (major or minor) when no traditional delivery program exists -Use Full Program Proposal

What information is needed to complete Appendix C?

New program (minor, area of concentration, or track)

  • program name
  • name of degree that student will earn after completing program requirements
  • title of proposed new minor/area of concentration/track
  • number of hours requires for new minor/area of concentration/track
  • additional resources required (need approval for Academic Planning Form from Provost prior to submission of form)
  • catalog information - should be as succinct as possible
  • semester and year program is proposed to begin
  • new code designator for SIS - if needed
  • delivery method - web-based/traditional
  • goals for proposed new minor/area of concentration/track

Program Change (Major, Minor, Area of Concentration, Track, or Web-Based Program

  • program name
  • name of degree that student will earn after completing program requirements
  • name of program to be changed
  • information for next catalog or requested change
  • semester and year change will take effect
  • if new or different code designator for SIS is needed
  • delivery method - web-based or traditional

Program Deletion

  • program name
  • when program will be deleted - semester and year

What browsers can I use to submit the forms:

You must be using Netscape version 4.5 or higher or Internet Explorer version 5.0 or higher in order for the curriculum forms to work properly.

What information is needed for each of the boxes on the form?

Subject - This box should contain a unique name for the form for each time you submit a form for same program.
Department/Unit Submitting Proposal: This box should contain the name of the department submitting the form.

Program Deletion, Indicate Program: Box should contain name of program to be deleted.

4. If Program Deletion Selected, Indicate Program: 

Narrative describing proposed action. Space is limited to 5000 characters and should not include any words in quotes such as "department" or 'department' as this tends to eliminate any information included after the quotes. If information is very lengthy, send as attachment to lepperm@exchange.nku.edu and information will be posted on web as a PDF document.

5.  Please provide a narrative that describes the action being taken with sufficient details for approval (limited to 5000 characters, if additional needed, please e-mail document as attachment to lepperm@exchange.nku.edu).

Proposed date for initiation of new program or program change/deletion. The boxes should contain the semester and year for implementation of proposed change.

The next section of the form should be completed only if a new area of concentration, track, or minor is proposed.

Name of Degree Earned: this box should contain the name of the degree that the student will earn when completing the program. Helps identify where the information needs to be updated for SIS, Degree Audit, and in the Catalog.

Title of Proposed Minor/Area of Concentration or Track. Box should contain the title of the new area of concentration, track, or minor. The information helps identify where the information needs to be updated for SIS, Degree Audit, and in the Catalog.

Number of Total Hours Required for Minor/Area of Concentration/Track. Box should contain the number of total hours required for the program. The information helps identify where the information needs to be updated for SIS, Degree Audit, and in the Catalog.

    12.  Number of Total Hours Required for Minor/Area of Concentration/Track

Catalog Information. Box should contain the information on the program to be included in next catalog. The paragraph should describe program, courses required, number of hours, and where to place information.
The following boxes should include information needed to update the SIS system and Schedule of Classes.

14. Do you need a new code designator (SIS) for the new minor/area of concentration/certificate program?   Yes   No

Additional Resources Required to Offer New Minor, Area of Concentration, or Track. The box should contain a list of resources needed for the program including item and estimated cost. If any additional resources are required that need to be funded by sources outside the department, the proposer must submit an Academic Planning Form to the Provost and receive approval prior to any action being taken on this form.

Minor, Area of Concentration, or Track program goals. The box should contain the goals specified for the proposed program. The information should be specific and may be used for the catalog.

NAME OF WEB-BASED PROGRAM - Department must have an already approved major/minor/area of concentration/certificate program/track prior to requesting approval for web-based program. All courses included in the program must be approved prior to implementation of new web-based program.

CATALOG INFORMATION - Indicate the information that should be included in the undergraduate/graduate catalog. Include such information as courses required, description of program, prereqs, admission requirements.

Avoid using quotes (both single and double) around words, this truncates the information from that point on. ADDITIONAL RESOURCES REQUIRED TO OFFER PROGRAM - Indicate any funds, equipment, space, library resources that are required to offer program.

GOALS/OUTCOMES FOR PROGRAM - Indicate specific goals/student outcomes for program. Are they similar to program being offered in a traditional delivery format?

All curriculum forms must be posted on the web for all UCC members and other interested parties to review. Box is automatically filled in with current date plus fourteen days.

27.   COMMENTS MUST BE RECEIVED BY:  Should be at least two weeks after form submitted.

 

 

Submission information and signature process. The curriculum form information will automatically be sent by e-mail notification to all UCC members and other interested parties. E-mail notification that the form has been submitted will also be sent to "original proposer." If for some reason when you click the "submit" box on the last form does not send information to the database and give you a "Thank you" message, it is possible that one of the boxes contains information that is too lengthy. If so, fax a copy of the form (second page of the form will give you a form that is printable) to Curriculum, Accreditation, and Assessment at 859-572-6055.

When you click the "Review/Print" button at the bottom of the first form, a second form will display a printable form. Print this form so you have a copy of the information submitted. The form is different from the first form and all the information is contained on lines instead of in boxes. Verify that the information is correct prior to printing. If correct, print the form and click the "Submit" button at the bottom of the form. This will send all the information to UCC members and enter the information into the UCC database. You will receive a "Thank you for submitting form" message and given a choice of three options:

After the form has been submitted, it is the responsibility of the original proposer to obtain the signatures required and to make sure that the form has completed the curriculum process. The original proposer should follow-up with the College Curriculum Committee chair to ensure that the proposed curriculum form is placed on the UCC agenda for vote.
